I think there is some confusion about some of the reasons, and actions taken for a ban.
Accounts are banned, because they present an active problem, or present a threat to income. Banning these accounts, even it only temporary, is very effective. Active problems are such thing as language issues, harassment, staff impersonation and such. Banning the account buys your time to deal with any technical issue that allows this, and creates a visible separation between the issue and a resolution. This is just as much a PR issue as an operational issue. Threats to income are such thing as credit card fraud, cheating, botting, etc. The ban here buys time to look into the matter to find more technical solutions, but it also sends a message that 'fairplay' is enforced by not just the game, but the staff. This again is as much a PR issue as it is an operational issue.

Banning is a last resort for the main reason you mention: if someone really wants to play your game, they will. Once you ban them, you lose track of them, because they will just return with a new account. Even worse, the more destructive someone is the more likely they are to return after a ban. This is why I always say the most foolish thing an MMO developer could ever do in trying to "stop" RMT is ban people who buy gold. The person who buys gold likes your game enough to well.... buy gold. They also clearly care a lot about their character's progress. If you ban them, they are faced with having to start over. That's not likely. But the gold farmer/frauder, he/she has to buy a new account in order to stay in a job. So you can be certain he/she will be back.

Since it is accounts that are being banned, perhaps it makes good business sense for a company to ban an account rather than impose an in game penalty. In this way, your first point contradicts your second point. Yes they are paying customers, but in the case of gold farmers, or even EULA violators, if they buy an additional account so they can continue to do whatever they were banned for, that is money in the bank.
For example, in this way, the developer shares in the gold farmer's profits. You ban the farmers, they buy additional accounts because even with the ban, they still make a profit, and the company derives additional revenue. Ban just enough to keep them coming back, but not so much that they are no longer profitable. Online identity may be tenuous, but if violators continue to pay to come back, banning is good business. It's good PR in that it gives the community a good feeling that the company is 'doing something' to combat the violators, and the company may derive additional sales. A win-win situation, is it not?
Actually, the problem with banning gold farmers is that since they are repeatedly being banned, they have switched over to credit card fraud, so they aren't paying for their accounts.
